Florida requires specific elements on all WDO-related proposals and service agreements under Florida Administrative Code Rule 5E-14.105. Below is what's required and where to set each item up in Smarter Launch.
Notice Banner
Every termite proposal must display one of the following notice banners on the first page of the proposal PDF. The banner text should be bold and 16pt font. Alternatively, the banner can be placed as the first item in the service agreement, in bold, 16pt font.

How to add in Smarter Launch: Go to Proposals > Templates and open your termite proposal template. Find the Notice Banner section and enter the appropriate banner text. The Notice Banner is designed for compliance language that must appear prominently on the first page of the proposal PDF with a bold box around the text.
Alternatively, the banner can be added as the first line of your Service Agreement in bold, 16pt font. The only difference is that it will not be on the first page and will not include a bold box around the verbiage. Go to Proposals > Service Agreements, open the relevant agreement, and place the banner text at the very top of the agreement body.

Florida Consumer Notice Form
All termite treatment proposals require the Florida Consumer Notice form. It must also be appended to the proposal template so it is included in the PDF sent to the customer.
How to add in Smarter Launch: The Florida Consumer Notice is available in the Media Hub under Documents. Go to Proposals > Templates, open your termite proposal template, add the form from the Forms section, and ensure it is also set to append in the Attached Documents section.

Florida Consumer Consent Form
The Florida Consumer Consent Form is required on WDO agreements when a termite warranty is transferring from one company to another.
How to add in Smarter Launch: Add the Florida Consumer Consent Form from the Forms section on the relevant proposal template. If it is mapped to a PDF, also add it in the Attached Documents section.

Notice of Subcontracting (Fumigation)
If fumigation services are subcontracted to another company, Florida Statute §482.227 requires a Notice of Subcontracting Form to be included with the proposal.
How to add in Smarter Launch: Add the Notice of Subcontracting Form from the Forms section on your fumigation proposal template or directly on the relevant service plan. If mapped to a PDF, also add it in the Attached Documents section. See Creating and Editing Proposal Templates and Mapping a Form to a PDF for more details.
