Arizona requires specific elements on all termite-related proposals and service agreements, regulated by the Arizona Office of Pest Management (OPM) at opm.azda.gov. Below is what's required and where to set each item up in Smarter Launch.
Important: All Arizona companies must have their termite treatment proposals approved by the state before use. Email a copy of your treatment proposal to Kellie Smith at [email protected] or call 928-300-7846. If she is unavailable, contact Alan Pugh at the Pest Management Division (PMD) at 602-542-4373.
Applicator Certification Number
The inspector's applicator certification number is required on all termite-related proposals.
How to add in Smarter Launch: Your certification number is pulled from your account profile. Go to Settings > My Account and ensure your applicator certification number is entered and up to date. It will then be available to display on your proposals.
Required Termite Treatment Information / Questions
All termite treatment service plans must capture the following information via a form or custom fields on the service plan. These fields collect the treatment details required by the OPM for each job.

How to add in Smarter Launch:
Via a Form (recommended)
Go to Settings > Custom Settings > Forms and create a new form.
Name it Termite Treatment Information
Add in your custom questions > hit save > add to your termite template(s)
Be sure to Allow Customer to View Form Results

Via Custom Fields in a Service Plan
Go to Proposals > Service Plans and open your termite service plan. In the service plan editor, add each of the above as a Custom Field. Custom fields are best used for a small number of questions specific to one service plan. See Creating and Editing Service Plans for more details.
AZ Pesticide Notice
The following pesticide notice must be attached as a Service Agreement to all termite-related service plans:

How to add in Smarter Launch: Go to Proposals > Service Agreements and create a new service agreement with the notice text above. Then attach it to each of your termite-related service plans in the service plan editor under Step 3.
