Purpose

Forms facilitate structured and systematic data capture, essential for a range of business operations. Depending on the assignment of the form, they can be tailored for various functionalities like Proposal Templates, Quality Assurance, or Service Plans.

Form Assignments

Depending on the operational requirement, a form can be mapped to the following areas:

  1. Proposal Templates: Structuring and detailing proposals.

  2. Quality Assurance: Ensuring service or product quality adheres to preset standards.

  3. Service Plans: Outlining the blueprint of services offered to customers.

Creating a Form

When you're in the process of creating a form, the following parameters demand attention:

  1. Form Name: Every form requires a distinct name for identification.

  2. Form Assignment: This is where you select one of the three operational areas for the form to be associated with.

  3. Unique ID: This identifier must be a combination of letters, numbers, and underscores. It aids in precise referencing.

  4. Filled Out By: Define who the primary user of the form will be:

    • Team Member

    • Customer

Form Fields

Each form demands at least one form field for effective data capture. These fields can span across:

  • Checkbox

  • Date

  • Multi-select

  • Number

  • Page

  • Radio Button

  • Section

  • Select

  • Text

  • Text Editor

For every form field, the following attributes are vital:

  1. Input Label: This display name clarifies the purpose or data expectation of the field.

  2. Input Type: Designate the desired format for data entry.

  3. Default Value: Pre-populated data that can be set for types 'Page' and 'Section'.

  4. Required Field Option: This sets the mandate for the field – whether users must fill it or can skip it.

  5. Help Guide: An optional attribute, this pops up as a '?' on the form. It offers users clarity or guidance on the data entry expected in the respective field.

Advanced Field Options

For Checkbox, Multi Select, Radio Button, and Select, you can:

  • Define Options: List out choices for these fields. Each option should be on its individual line and enclosed in quotes. For instance:

    • "Yes"

    • "No"

  • Default Selection: For a pre-selected option, append the ":selected" suffix:

    • "Yes":selected

    • "No"

  • Setting Response Score: Assign scores to input options using the ":score" suffix:

    • "Yes":selected:score(1)

    • "No":score(0)

  • Display Comment Field: Showcase a comments section if an option is chosen by adding the ":comments" suffix:

    • "Yes":selected:score(1)

    • "No":score(0):comments

Rules for Forms

These rules are applied on a per-company basis within the Smarter Launch application. While each company must adhere to these rules within its own settings, the rules do not extend across different companies.

  1. Unique Form Names: Each form should have a distinct name.

  2. Distinct Field Labels & Types: A field's label combined with its input type should be one of its kind within a form.

  3. No Deletion if In Use: If linked to any assignments, the form shouldn't be discarded.
